At the recent MedCity News "Bullseye" conference in Chicago, industry leaders gathered to dissect this dichotomy. The consensus was clear: the American healthcare system is failing at the delivery of care, trapped in a cycle of opaque pricing, logistical deserts, and fragmented benefit structures.
The Reality of Rationing: A Crisis of Affordability
The human cost of the current pricing structure is staggering. According to Jay Rughani, a partner at Andreessen Horowitz, the financial burden placed on the average consumer has reached an unsustainable breaking point.
"A quarter to a third of adults skip or ration their medications because of the cost," Rughani stated while moderating the panel. "We find this unacceptable. This is increasingly putting pricing pressures on the demand side, on the employer side, on the payer side, and on the government side of how we’re thinking about making these medicines affordable."
For many, the choice between paying for a prescription and paying for rent is not a hypothetical scenario, but a monthly reality. When patients ration doses, they risk their long-term health, often leading to emergency room visits and hospitalizations that cost the healthcare system significantly more than the medication itself.
The Friction Points: Logistics and Opacity
The barriers to medication access are not solely financial; they are deeply structural. Tanvi Patel, VP and General Manager of Amazon Pharmacy, highlighted that even for those who can afford their prescriptions, the physical act of obtaining them is often a hurdle.
The "Pharmacy Desert" Phenomenon
Patel pointed to Seattle—a city known for its technological infrastructure—as a prime example of a pharmacy desert. In many urban centers, residents must travel long distances to reach a retail pharmacy. For an elderly patient or a working parent, a 10-mile trek and a one-hour time commitment to pick up a prescription can be the difference between adherence and abandonment.
The Transparency Deficit
Beyond geography, the lack of price transparency acts as a "black box" that prevents informed consumer choice. "Consumers do not know how much they will need to pay before they reach the pharmacy," Patel explained. By the time a patient learns the price at the counter, the inconvenience and the cost shock often lead them to walk away empty-handed.
AJ Loiacono, CEO of Judi Health, argued that this lack of transparency is not a bug in the system, but a feature. "When people don’t understand the appropriate price, they overpay," he noted. Loiacono describes the pharmaceutical market as a "tug-of-war" between manufacturers and payers, where the government—the largest purchaser of drugs—fails to present a unified front.
The government’s pricing structure is fractured across the 340B program, Medicare, Medicaid, the Department of Defense, and the Veterans Administration. If the federal government cannot leverage its massive scale to secure a single, rational price, the prospect for small businesses—like local plumbing or towing companies—is bleak. "They have zero chance," Loiacono added. "They are the ones that are being absolutely abused under this system."
Chronology: The Evolution of Market Complexity
The current state of the drug market is the culmination of decades of incremental legislative and commercial changes.
- Pre-2000s: The pharmaceutical industry operated under a more traditional, albeit still opaque, fee-for-service model.

Annie Collins, Chief Commercial Officer at Aradigm Health, highlighted that many of the most innovative, high-cost treatments—specifically cell and gene therapies—are administered in clinical settings. These are not "pill-bottle" drugs; they are complex biological interventions.
"These are drugs that are administered by doctors and, for the most part, actually procured by doctors as well," Collins explained. "You’re not going to a pharmacy to pick up your medical benefit drugs. It’s mostly providers and payers having a little battle in the background."
The "Habit" Problem in Clinical Selection
Even when multiple options exist, market forces do not always prioritize the most cost-effective solution. Collins cited the recent FDA approval of Casgevy (Vertex Pharmaceuticals) and Lyfgenia (bluebird bio) for sickle cell disease.
Despite Casgevy demonstrating superior clinical outcomes and a price point $900,000 lower than its competitor, many providers continue to prescribe Lyfgenia. Collins attributes this to clinical inertia: doctors are prone to prescribing the therapies they worked with during clinical trials, prioritizing familiarity over economic logic. This behavior creates a market where clinical outcomes and cost-efficiency are decoupled.

Industry leaders are calling for a fundamental restructuring of how drugs are priced and distributed.
The Push for Transparency
Amazon Pharmacy is advocating for a retail-style transparency model. By allowing consumers to see prices upfront, they believe the market will naturally favor companies that offer better, more efficient experiences. However, experts like Patel acknowledge the limitations of existing attempts at transparency, such as the TrumpRx website. While helpful, it focuses on cash prices for a limited list of generics—a solution that only touches the less than 10% of Americans who pay for their medications in cash.
The Call for Pre-Launch Negotiations
Collins argues that the industry must move beyond reactive pricing. With approximately 2,000 cell and gene therapies currently in the development pipeline, the current "wait-and-see" approach to pricing is unsustainable. Pharmaceutical companies must engage with payers to determine sustainable, value-based pricing before drugs reach the market.

The implications for the future of American healthcare are severe. If the current trajectory continues, we face a "bifurcation of care": a future where only the wealthy or the heavily insured have access to the miraculous, curative breakthroughs of modern medicine, while the elderly, the poor, and the employees of small businesses are left to ration older, less effective treatments.
The transition from a system that treats symptoms to one that offers cures is a medical triumph, but it is currently a financial catastrophe. As the panel concluded, the science is moving at a breakneck pace, but the policy and distribution infrastructure is stuck in the 20th century.
